A program of providing nutrition allowance of Rs.20000 pregnant mothers

Comply with the application form from the Clinic center / Divisional Secretariat that you attend, confirm your residence from the Grama Niladhari of the permanently settled Grama Niladhari Division and recommendation from the Family Health Services Officer and the clinic in charge of the clinic in charge of the maternity clinic, before 3 months pregnancy. Recommendation of scholarship applications

 Purpose

 Purpose

Public Assistant & Disease Assistant

Purpose
Providing dependent allowance for families requiring low income and special concessions

Required Qualifications
• Monthly Income Rs. Less than 6,000
• Aging
• Physical and mental disorders
• Widows, abandoned by their master, women who are incurable and imprisoned
• Be orphaned and physically mentally disabled children under 16 years

Disease Assistant

Purpose
For low income patients with a specific ailment

Required Qualifications
• Being a member of a low income family
• Tuberculosis, Cancer, Thalassemia, Leprosy and Kidney disease

• Medical certificate on relevant disease
• Application
• Grama Niladari report to prove low income and residency
• Expert medical certificate for kidney disease
• Recommendation of the Social Services Officer

Obtating a Prior Right

  1. 1. Request letter
    2. Report of Grama Niladhari
    3. A photocopy certified by the original deed
    4. Relevant license plate (from Mathugama / Horana Land Registrar)
    5. Certified death certificates of the grant holder and the spouse
    6. If the post inheritance has been named, his birth certificates and photo copies
    7. If the post inheritance is not named and if the invalidity certificate is not available, the certified copies of all his brothers and sisters
    8. If there is no succession inheritance or if the applicant is married, copies of the certified copies of his children's certificates
    9. Affidavit if there is a difference in names
    10. Individuals should come to the National Identity Card and make statements according to above number 7/8.

Disposal of the Land by Notary Deed

  1. 1. Request letter
    2. Report of Grama Niladhari
    3. A photocopy certified by the original deed
    4. Passport relevant to the grant (obtained from Matare / Horana Land Registrar)
    5. A copy of the deed of proof that the landlord has other lands (certified by the Grama Niladhari) and its post
    6. If a part of the land is excluded, a plan and a photocopy
    7. A photocopy of the certified marriage certificate from the original
    8. Certified Photocopies of children's birth
    9. Certified Birth Certificate Certificates from the original to prove the relationship
    10. Affidavit if there is a difference
    11. The gift certificate and the spouse should come with the declaration of the children more than 16 years old with the National Identity Card.
